Eligibility Requirement:

  • Have less than $3368/mos unearned or earned gross income (for one person) per month.
  • Must apply for Medicaid first.
  • Must complete application for MDCH, which includes statement on your medical status.
  • Private insurance copays may be covered if financially eligible.

Benefit Amounts:

Formulary covers 175 HIV and non-HIV meds and vaccines. Call (888)826-6565 or (877)FIA-AIDS for complete drug list.

Payment Intervals:

Payment is made to the pharmacy at the time of filling prescriptions.

Duration of Coverage:

  • Coverage is ongoing as long as funds are available.
  • Eligibility is reviewed yearly.

    How to Apply:

    Contact your local case manager or your local Family Independence Agency office.

    Note: Eligibility and the extent of coverage varies according to the availability of government funds.
