- Must apply for Medicaid first.
- Must complete application for MDCH, which includes statement on your medical status.
- Private insurance copays may be covered if financially eligible.

Payment Intervals:
Payment is made to the pharmacy at the time of filling prescriptions.

- Coverage is ongoing as long as funds are available.
- Eligibility is reviewed yearly.
