- Must apply for Medicaid including a statement of medical status.

Benefit Amounts:
The program covers the full scope of dental services but does not cover orthodontics, braces, implants, or cosmetic denistry.

Payment made at the time of service.

Eligibility is reviewed yearly. Coverage is ongoing while funds remain available.
